I have them in as soon as I step foot in the venue if there is an act before me, and I have them around my neck during show and pop them in between mixes if it is a loud environment.
Nothing beats molded earplugs. Otherwise these Etymotics are popular:

IMO, you're better off getting ear plugs and using a standard on ear/over the ear headphone.
