Making A Legit Press Kit/CV

So I have photos, video (if I can find it somewhere), fliers, recordings of live sets, mixes, and need to make a nice resume so that I can finally get my lazy ass into some clubs. Anyone have any good links to share or advice (or even better would someone be willing to send me their press kit as an example?)
